A reminder that students who are playing a sport for Cloquet High School or Middle School in grades 7-12 need a current MSHSL sports physical form on file before they are allowed to try out for fall sports . Physicals are good for three years; most incoming 7th and 10th graders will need to get physicals this summer. Any student without a sports physical on file will not be allowed to practice or compete.
Attached is the physical form that needs to be filled out by a doctor. The form is also available at https://www.isd94.org/Page/1180. CMH Raiter Family Clinic is offering free sports physicals on June 18. Pre-register at www.cloquethospital.com/happenings.
